The Origin of CladeHealth™ Tracker

CladeHealth Tracker originated in a chance discussion between two health care providers who realized that a better mechanism was needed for individuals to be able to have control of, and become empowered by, their medical information. These two clinician/researchers worked diligently with Aegis Creative to develop a product that is game-changing for both the person holding their information and for those who are allowed to add to and review it.

What Is a Personal Health Care Record (PHR)?

A personal health care record, or PHR, is typically an electronic record that collects and stores health data, initiated and maintained by an individual. Note: a PHR is not the same as an electronic health record (EHR) or a software system that a hospital or healthcare provider might use to document legally mandated notes on care that is provided. PHRs can contain a wide variety of information and data. See CladeHealth Tracker for the information and features currently provided.

Why a Personal Health Care Record?

With the introduction and widespread use of smartphones and the development of easy-to-use apps, it makes sense to have an application that can store, track, and help manage one's health—wherever you are and wherever your family or loved ones might live.

Given the trend from physician-only care to self-management and partnerships with multiple healthcare providers—as well as the evolution away from static charts and paper—a tool that provides actionable information at the point of personal healthcare decision-making is becoming an absolute necessity for healthcare consumers.

Why the Name Clade?

A clade by definition is a complete taxonomic group that includes all descendants of one common ancestor. The word is derived from the Greek klados meaning branch, which is the conceptual basis for CladeHealth Tracker name. The use of the word clade for this and future applications signifies a branch or aspect of health (for example, medications, conditions, labs and procedures) as well as disease management (for example, ophthalmic, cardiac, neurologic). CladeHealth Tracker is visualized as the core or tree, with the various elements associated with our health extending out from it (branches), organizing the clades of our biological information.
